Knowing who to contact during an emergency can save the life of your elderly loved one. Knowing which physician to contact, what medication to administer, and what insurance information to provide can be complicated due to the sheer amount of information you need to provide.   • Copies of medicare and supplemental insurance cards
  • Names and phone numbers of doctors
  • Contact information for anyone else they may need contacted
  • List you as a contact with the doctors and insurance companies
  • Living Will (advanced medical directive)
  • Power of attorney
